According to OSCA Continuing Policy, all employees must be evaluated on their job performance once a semester.
Your opinions are being solicited by the Personnel Committee to assist with evaluating employees with whom you have worked. To make sure the evaluation results are accurate and thorough, please answer all questions as truthfully and completely as possible. Please remember to evaluate the quality of job performance based on contractual job description, rather than the personality of the employee. The Personnel Committee realizes that, to varying degrees, any employee's personality affects their job performance. For the sake of objectivity, however, please try to distinguish between personality and performance, and fact from hearsay.
